Personal Data Collection Consent Form

This consent form explains how ESi Technology Ltd. (the “Company”) will handle the personal data collected through this form. By clicking “Submit”, you confirm that you have read, understood and agree to all of the terms of this consent form and any subsequent amendments. If you are under twenty years of age, you may use this service only after your legal representative has read, understood and agreed to all of the terms of this consent form and any subsequent amendments. However, if you have already accepted this service, you are deemed to have obtained the consent of your legal representative and to comply with all of the provisions below.

1. Collection, updating and safekeeping of personal data

The Company collects, processes and uses your personal data in accordance with the Personal Data Protection Act of the Republic of China (Taiwan) and related laws and regulations.
Please provide your own correct, current and complete personal data when you apply.
The personal data the Company collects in the course of its business includes your name, telephone number, email address and similar information.
If any of your personal data changes, please proactively ask the Company to correct it so that it remains correct, current and complete.
If you provide incorrect, false, outdated, incomplete or misleading data, you will lose the related rights and interests.
Under the Personal Data Protection Act of the Republic of China (Taiwan), you may exercise the following rights regarding your personal data: (1) to inquire about or review it; (2) to request a copy; (3) to request supplements or corrections; (4) to request that its collection, processing and use be stopped; and (5) to request its deletion.

2. Purpose of collecting personal data

The Company needs to collect your personal data to meet its business needs.
If your personal data is to be used in a way that differs from the purpose for which the Company originally collected it, we will ask for your written consent before such use. You may refuse to provide your personal data to the Company, but you may lose your related rights and interests as a result.
The Company will use your personal data for one year starting from today, within the Taiwan area, and will regularly destroy the completed application forms and consent forms in the following year.

3. Confidentiality of personal data

If your personal data is stolen, leaked, altered or otherwise infringed because the Company violates the Personal Data Protection Act, or because of a natural disaster, incident or other force majeure, the Company will, after investigating the matter, notify you by an appropriate method such as telephone, letter, email or a notice on its website.

4. Effect of this consent form

By clicking “Submit”, you confirm that you have read, understood and agree to all of the terms of this consent form. If you violate the following terms, the Company may terminate all rights, interests or services provided to you at any time.
The Company reserves the right to amend the terms of this consent form at any time. When it does, the Company will announce the amendment on its web page (website) and will not notify you individually. If you do not agree with the amendments, please stop using this service. Otherwise, you will be deemed to have agreed to and accepted the amendments or additions to this consent form.
No advice or information, whether written or oral, that you obtain from this consent form constitutes any warranty other than those expressly set out in the terms of this consent form.

5. Governing law and jurisdiction

The interpretation and application of this consent form, and any disputes relating to it, shall be governed by the laws of the Republic of China (Taiwan), and the Taiwan Taipei District Court shall have jurisdiction.
